Announcement Regarding the Closure of Infinity Pediatric and Adolescent Medicine
It is with profound sadness that I announce the closure of Infinity Pediatric and Adolescent Medicine.
Due to a significant decline in my health, my physicians have advised me to step away from practicing medicine and focus on spending the time I have with my family. While this decision was not made lightly, it is necessary at this stage of my life.
It has been both an honor and a privilege to care for the patients and families of Infinity Pediatric and Adolescent Medicine and to be a part of the Smithfield community. I am deeply grateful for the trust you have placed in me over the years, and I will miss serving this community more than words can express.
In accordance with Virginia law, our office will remain accessible for 60 days following this announcement. However, we will discontinue patient care services after September 18, 2026. During the required accessibility period, the office will remain open solely to assist patients with obtaining their medical records.
If you need a copy of your medical records, please complete a medical records request form. Once the form has been submitted, records may be picked up in person, or we can fax them directly to your new healthcare provider if they are able to receive faxed records. Unfortunately, mailing records is not an option at this time.
We also encourage you to contact your insurance provider as soon as possible to select a new primary care provider (PCP).
Thank you for allowing me to be a part of your lives and your children’s healthcare journey. It has been a privilege to care for you, and I wish you and your families continued health and happiness.
With sincere gratitude,
Dr. Sandra Baucom
