Life is busy. Juggling your day-to-day schedule and staying on top of your family’s health care can be a challenge, but our patient portal can help. Access your child’s medical records 24/7 from your home, office or smart phone.

Convenient Access to Health Records

From the patient portal, you can review a complete health information summary for each child in your care, including:

Most recent well child visit date
Upcoming appointments
Historical visits and visit summaries
A summary of labs and medical tests
A problem list, allergy list and medications list
A complete immunization record, available to download
Growth parameters, available to download

Secure Communication

Secure messaging is a feature of our patient portal. You can use it to contact us about non-urgent medical questions, medication refills, and appointment requests.

We will communicate with you via the portal for lab results and non-urgent communication in order to link all communication within your child’s chart.

Need to sign up?

We encourage all families to sign up for the portal to help us be sustainable and paperless. You will receive an invitation to sign up for the portal once your child is registered.

Important Instructions for Proper Use

Always read and follow the label instructions.
Whenever possible, use the dosing device that came with the product. Syringes and droppers are more accurate than teaspoons. If you use a teaspoon, it should be a measuring spoon. Regular spoons are not reliable.

Acetaminophen (Tylenol)

Not for use under 2 months of age unless instructed by a healthcare provider.
Repeat dose every 4-6 hours while symptoms last.
Do not give more than 5 doses in 24 hours.
Do not use with any other product containing acetaminophen.

Ibuprofen (Advil, Motrin)

Not for use with children under 6 months of age, unless instructed by healthcare provider.
Repeat dose every 6-8 hours as needed.
Do not give more than 4 doses in 24 hours.

Diphenhydramine (Benadryl)

Not for use with children under 6 months of age, unless instructed by healthcare provider.
Non-sedating alternatives (eg, loratadine, cetirizine, fexofenadine) are available over-the-counter and are safer for young children.
Repeat dose every 6 hours as needed.
Children’s Benadryl Fastmelts: Each fastmelt tablet contains the equivalent of 12.5 mg of Diphenhydramine HCL, dosed the same as chewable tablets.
Risk of side effects: May cause drowsiness and paradoxical excitatory. Use caution when driving or operating heavy machinery after dosing due to potential for sedation and decreased alertness. This is especially relevant for teen drivers.

Tips for Your Virtual Visit

Good lighting is helpful. Place the light in front of you and your child, rather than behind you.
Please have your child’s recent weight and temperature.
We may need to review any medications your child is using, and verify your preferred pharmacy.

Privacy Considerations

Please understand that while this software is HIPAA-compliant, by choosing to do a virtual visit, you are agreeing to electronic communication of you or your child’s health information through audio, video, and/or photo technology. While our software is secure, consider any privacy needs in your space as well.

CHADIS Pre-Visit Questionnaires

CHADIS is a web-based screening, diagnostic and management system that administers and analyzes pre-visit, online questionnaires completed by parents, teens, and teachers, and provides us with instant access to valuable clinical data and resources.

CHADIS questionnaires are an integral part of well child and behavioral health visits, and help us to provide the highest quality of care. They are available online through our Patient Portal and need to be completed prior to your child’s scheduled appointment. For children age 12 years and up, both a parent and child questionnaire need to be completed.

Most insurance companies cover CHADIS questionnaires; however, it is your responsibility to verify coverage with your insurance company. If any questionnaires are denied by insurance, the cost will be your responsibility.
